I went the insurance route in 2016, and at the time in NY you had to be one year on testosterone before getting any surgery.

I was treated great. Hospital staff were friendly and the doctors were amazing. I was the first trans person the surgeon operated on and I am happy about the results. I did have to get a revision to resize the nipple but other than that, great experience.
